How Much Money Does a Gestational Surrogate Make in the U.S.?

Let’s start with the big picture. In the United States, how much gestational surrogates get paid depends on several factors:

  •     Is it your first time as a surrogate?
  •     Do you have a surrogacy-friendly insurance policy?
  •     Are you carrying twins, or planning to pump breastmilk?
  •     What state are you delivering in?

On average, first-time surrogates can expect to earn between $50,000 and $55,000, while experienced surrogates may earn $60,000 to $75,000 or more, depending on their circumstances.

What Is Base Compensation?

Your base compensation is the total amount you receive during the pregnancy. It is typically paid monthly starting around the confirmation of heartbeat (around 6-7 weeks of gestation), and it continues monthly through delivery. Base compensation is most commonly disbursed in 8 to 10 installments, and the number of installments may vary slightly, but the principle remains the same: predictable monthly payments that honor your ongoing commitment to keeping the Intended Parent’s baby as safe and healthy as possible.

If you already have a surrogacy-friendly insurance policy, reviewed by experienced surrogacy professionals, you may be eligible for a higher base comp. That’s because your intended parents won’t need to purchase an entirely new policy, and your health insurance will be able to be used for the journey.

This base compensation acknowledges your physical, emotional, and logistical commitment. It’s the financial foundation of the journey, but not the whole picture.

Surrogates receive detailed surrogate financial packages, tailored to their needs and journey preferences. These compensation extras are outside of the base compensation and are clearly outlined as milestone payments for the duration of the journey.

What Are Surrogate Bonuses and Reimbursements?

As a surrogate, you are eligible to receive additional payments are sometimes called “bonuses”, “allowances”, “fees”, and/or “expense reimbursements”. While the terminology may vary, the goal is to fairly recognize the milestones, procedures, and additional needs that arise during the surrogacy journey. We use the word “fees” here because it is widely recognized by prospective surrogates searching for answers about surrogate pay. Some bonuses are not guaranteed (as they depend on specific circumstances), but many surrogates do receive several of them. Medical & Procedure Bonuses

  •     Mock cycle / Trial IVF cycle fee (if required by the Intended Parent’s fertility clinic)
  •     Embryo transfer fee (this is on top of lost wages and travel expenses)
  •     Start of medication fee (when you start IVF medications like daily injections, patches, hormone pills, etc, to prepare for embryo transfer)
  •     C-section delivery fee (if medically necessary)
  •     Invasive procedure fee (amniocentesis, cerclage if medically necessary)

Pregnancy-Related Bonuses

  •     Multiple fetus pregnancy (if one embryo splits into two/three babies)
  •     Maternity clothing (paid once as an allowance for the journey)
  •     Termination/reduction fee (this is rare, but sometimes unavoidable)
  •     Loss of or Damage to reproductive organs (this is also rare, but sometimes unavoidable)

Monthly & Lifestyle Support

  •     Monthly allowance (for small incidental items like prenatal vitamins, monthly mileage reimbursement up to 100 miles, short childcare needs so you can go to a local appointment, etc.)
  •     Housekeeping allowance (paid once towards the third trimester, or if you are on prescribed bedrest)
  •     Childcare during medical screening/transfer travel, and/or prescribed bed rest
  •     Surrogate Support for Mental Health needs, doula, and chiropractic care
  •     Breastmilk pumping (a weekly allowance for the duration you choose to pump breast milk if you are open to doing so. The pump, supplies, and shipping expenses are a separate cost for the intended parents to cover)
